All Incoming Students

 

All new students including (Continuing, Transfer & Graduate and Post-Baccalaureate) beginning classes during the Spring and Fall 2024 ºÚÁÏÍø term students are required to comply with California State University system wide Executive Order 803, including the following immunizations:

Measles, Mumps and Rubella (MMR)

All incoming students born after 1956

  • 2 doses on or after 1st birthday
Meningococcal Conjugate (Serogroups A,C,Y & W-135) (MenACWY)

All incoming students through 21 years of age

  • 1 dose on or after the 16th birthday
Hepatitis B

First-time enrollees at California State University campuses 18 years of age and younger.

  • Full Immunization series

 

Uploading Immunization Records to the student portal.

Visit single sign-on webpage:  â€‹ and click on Student Health Services.

  • Choose the "Student" option.

  • Enter your 9 digit Campus ID Number.

  • Choose "REQUIRMENTS" from the navigation menu.

  • Click Update on the vaccine you are adding information too.

  • Add Vaccine History and any Test Results as needed then click Done.

  • Upload all supporting Immunization Documentation. (This can be done from either location listed below)

Records may take up to 7-10 business days to process after they have been submitted to the Patient Portal.

Periodically check to see if you have Satisfied the immunization requirements in the Clearance section of your Patient Portal.

If it still says Not Satisfied after 7-10 business days check the Messages section for any messages about missing requirements.

Make sure your name and date of birth is located on every page of your immunization record. It is a legal document. Your scanned document will not be reviewed if it's missing this information.

All immunization records are required to be translated to English to be reviewed.
